Hi Clipperwood,

sorry for the delayed reply, I've been on holiday.

The Aware OMT test is based on detection of antibodies against HIV-1 gp41 and the HIV-2 ortholog gp36 (both proteins, in recombinant form, are represented in the Test Zone of the Aware strip).

To the best of my knowledge, all known HIV-2 subtypes generate cross-reactive antibodies; that is, to the extent that gp36 differs among HIV-2 subtypes, it does not differ sufficiently that antibodies raised against one subtype fail to recognize any of the others.  (This may prove not to be the case if more HIV-2 subtypes are discovered, or even as we learn more about the subtypes already recognized.)

HIV-1 subtypes vary more widely than HIV-2 subtypes do, but gp41 is a highly conserved molecule and it is generally accepted that antibodies raised against gp41 from any of the M group HIV subtypes will cross-react with gp41 from any other M group subtype.

Thus, I think it accurate to say that the Aware OMT test can detect all known HIV-2 and M group HIV-1 subtypes.

HIV subtypes N and O, however, show considerable divergence from the M group, and it is possiblethat some antibodies raised against subtype N or O gp41 would not recognize the gp41 used in the Aware OMT test.

We have tested a small number of known, archived HIV type O positive specimens, all of which were detected by Aware OMT (known group N specimens are so rare that we have never had the opportunity to test any).  We have also conducted a small clinical trial in Cameroon (home to nearly all the type O and type N infections identified to date) and although no subtype information was obtained, we did not see significant evidence of lower sensitivity or specificity in that population than in any of our other trials -- see http://www.ncbi.nlm.nih.gov/pmc/articles/PMC2695602/.  This is good (though not definitive) evidence that Aware OMT can indeed detect non-M-group HIV-1 subtypes.

All best,

Bill.

Hello,Bill
It's really very warm-hearted for you to spare your valuable time to answer my question. In fact, i have telephoned the similar questions to your china company, unfortunately they showed less professional knowledge than you proved to be. Pls receive my respect for you. In the former Email you have sent to me, you told me that under the circumstances of colder temperature, it might take long-time for the appearance of a weak positive result. I will not simply think of your answer as an over-conservative one and i have just bought another OMT device through Internet.

These days i have visited some websites you've recomended to me, and i discovered some basic knowledges concerning HIV/AIDS. I have known that HIV is divided into 2 groups(HIV1 and HIV2) and there is also some subtypes of HIV 1. Would you think me impertinent if i were to ask you whether your OMT device detect all subtypes of HIV1 or HIV2?what puzzles me that is some scientist claims to find the existence of HIV O(outgroup). Though i know is quite rare and maybe it is only found in western africa, i am quite curious to listen to your advice. Do your device can detect HIV 0?

Best regards,

Clipperwood
