查了好些资料。

这样还是说明抗体会在6-8周内达到巅峰值,其实也是说明6周检查的意义是相当巨大的。对心理恐惧而身体正常伴有症状的人来说,已经足够了。

是这样的,特别是6周去做4代会有非常大的意义,因为根据图上的说法四代可以缩短窗口期7-12天。

不同意你的说法,如果是看图说话,抗体在一个月的时间内,是很少量的,而7-12天,这个时间最多是个急性症状期,而抗体很少,不容易检测到。即使是4代酶免,在没有抗体的情况或者很少的情况下,也是意义不大的检查。
所以还是得4周之后的检查,才是大局已定的结果。

4代可以测p24抗原。上面的图也有可能是作者按3个月做的正太分布图,不一定准。

以下纽约周的说法:
New York State Recommendations for HIV Testing and Re-testing
1. What is the Window Period?
HIV is most commonly diagnosed in adolescents and adults through HIV antibody testing. The window period is the length of time after infection that it takes for a person to develop enough antibodies to HIV to be detected by current blood or oral fluid HIV antibody tests. When an individual becomes infected, his or her body will develop antibodies against HIV. Once enough antibodies are developed, the HIV antibody test result will be positive. Most people who become infected with HIV will develop enough antibodies to be detected by current HIV antibody tests 3-6 weeks after the exposure. Each person’s body responds to HIV infection a little differently, so the window period varies slightly from person to person. It is unusual for an HIV-infected individual to not develop antibodies by 3 months after the suspected exposure. A person who tests HIV antibody negative 3 months after an exposure does not require further testing unless he/she has had repeated exposures or if the antibody test results are incompatible with the person’s clinical history.

他也是说3-6周抗体浓度足够而已,测抗体足够,测抗原国内一般还是以测抗体为主。
抗原,也许2周可以检测到,但是还是要看感染的方式。

对于产生抗体较慢的那部分人,测抗原还是有一定意义的,虽然抗原只能作为辅助。

Based on the assumption that HIV RNA is first detected approximately ten days after exposure, researchers have estimated window periods to be as follows.

[*]First detection of HIV RNA: approximately ten days after exposure (typical range 7 to 21 days).
[*]First detection of p24: approximately 17 days after exposure (typical range 13 to 28 days).
[*]First detection of antibodies: approximately 22 days after exposure (typical range 18 to 34 days).
However, these are averages, and if the period between exposure and detectable viraemia is as variable as some authors suggest, these periods will occasionally be several weeks longer.

Early Detection and the Window Period

Specific antibody to HIV is produced shortly after infection, but the exact time depends on several factors, including host and viral characteristics. Importantly, antibody may be present at low levels during early infection but not at the detection limit of some assays. Using the early-generation tests, antibody could be detected in most individuals by 6 to 12 weeks after infection. Newer-generation assays, including the third-generation antigen sandwich assays, can detect antibody at about 3-4 weeks after infection.(2) This window period before the detection of antibody can be shortened by several days using antigen tests, and by several more days using nucleic acid detection methods.(3) Therefore, in most individuals, the window period may be only 2-3 weeks if an all-inclusive testing strategy is used. Most antibody tests currently on the market have near perfect and equivalent degrees of sensitivity for detecting most individuals who are infected with HIV (epidemiologic sensitivity), but they vary in their ability to detect low levels of antibody (analytical sensitivity), such as those occurring before complete seroconversion.(2) Although tests are available to detect specific HIV immunoglobulin M (IgM) antibody, these tests have shown little utility in identifying early infection because IgM responses to HIV are not produced consistently during early infection.(4) The ability of some tests (eg, third-generation tests) to detect IgM antibody simultaneously with immunoglobulin G (IgG) detection, however, may be responsible for their higher analytical sensitivity.

嗯,6周是在现今科技免疫学检测方法达到一定水平得出来的。
我个人也觉得除了癌症晚期患者、接受化疗的患者。、吸毒者。接受完器官移植手术,服用抗排斥药物的患者。自身存在严重免疫系统疾病的患者。其他的人6周绝对是可以定心了,没有任何问题。
如果是测抗原的话,那么是可以更早一些,抗原是比抗体早出现的,毫无疑问。
